Shaken Baby Syndrome (SBS)

By now most of you have heard about the story of Baby Kaleb Schwade circulating Myspace. He is in a hospital recovering from SBS, Shaken Baby Syndrome. The verdict in this case is still out so I do not and will not speculate on the innocence of the woman Kaleb was left with when he became ill. Because Kaleb is NOT the only child with SBS, I think SBS is a very serious issue that needs to be addressed! There are two sides to Shaken Baby Syndrome one is the physical abuse of babies. The other is a very serious medical illness. 

    

The abuse is an infant or small child being shaken or thrown into the air too roughly. Instead of rocking an infant who can't stop crying, the caregiver shakes the baby excessively resulting in unconsciousness. What actually happens is the child's head rotates uncontrollably because the neck muscles are not yet developed and provide little support. The movement makes the skull move around so much that blood vessels and the nerves of the brain rupture and the brain tissues tear. When the brain hits the sides of the skull, it causes bruising and bleeding to the brain. Of course the injury is more when the child's head is hit against a hitting a wall or a crib mattress. The brain then swells and puts pressure on the skull, which causes blood vessels to be compressed.

In America alone, yearly, an estimated 1,200 - 1,400 children are shaken. Twenty-five to thirty percent die as a result of their injuries. The rest have lifelong complications.

Symptoms according to the National Center of Shaken Baby Syndrome are:
· Lethargy / decreased muscle tone
· Extreme irritability
· Decreased appetite, poor feeding or vomiting for no apparent reason
· Grab-type bruises on arms or chest are rare
· No smiling or vocalization
· Poor sucking or swallowing
· Rigidity or posturing
· Difficulty breathing
· Seizures
· Head or forehead appears larger than usual or soft spot on head appears to be bulging
· Inability to lift head
Many SBS victims show any external signs of trauma and therefore go unnoticed. If they are noticed, the child must be taken to the hospital immediately.

The Aftermaths of SBS can be one or a combination of the following:
· Deafness
· Blindness
· Paralysis
· Coma
· Learning problems
· Delays in development
· Can not swallow
· Breathing problems
· Death

Doctors make the diagnosis by getting a detailed medical history and a physical exam. Blood tests are taken to detect bleeding problems, X-rays, CT and MRI scans of the head. Spinal fluid is checked for meningitis by doing a lumbar puncture. Shaken Baby Syndrome requires hospitalization, sometimes in an intensive care unit. Brain swelling or lack of oxygen to the brain, are complications that require immediate treatment. Depending on the effects of the brain's injury, oxygen therapy and/or a ventilator may be used to help the child breathe. The child may be given medication to help reduce swelling in the brain. A cooling mattress that helps to lower the child's body temperature and reduce brain swelling may also be tried. A child who has severe bleeding may need surgery to drain the fluid.

Commonly, SBS is a total injury to an undeveloped brain having minimal stored information. Therefore, the brain's adaptive abilities are severely impaired as well as the development of language, vision, balance, and motor coordination. Treatment is physical and occupational therapy to acquire the skills that would have developed on their own. Therapists do this through exercises that force the child to pay attention. They use sound and other stimuli to increase the child's interest in objects, such as repeatedly squeaking a toy near the child's ear. Some schools are providing information and developmental assessments for children under the age of 3. Children with SBS may require special education and continued therapy to help with language development and daily living skills such as dressing themselves.

Except for a handful of lucky ones, SBS is a life sentence consisting of various disorders such as seizures, which are sudden bursts of abnormal electrical activity in the brain, uncontrolled muscle movement, spasticity, mental retardation, learning and or behavior problems.

Interaction between human beings should never be harsh or abusive especially with babies. Hug them, rock them, love them – NEVER shake them!

A few small, easy changes in the products you buy and use can help reduce your child's exposure to toxic chemicals.

At the store

Choose Safer Toys and Teethers

~Look for "PVC-Free" on the labels or soft plastic toys and teethers. Another class of chemicals shown to disrupt the hormone system- phthalates- is found in polyvinyl chloride (PVC) plastic. PVS plastic is used to make different types of children's  products. including some teethers and soft plastic toys. Some manufacturers have removed PVC from their children's products, especially products intended to be put into children's mouths.

~Unfortunately, no law requires or regulates these labels, and few products are labeled as such. When parent's have a question about the chemicals in a product, they should call the manufaturer.

~Choose wooden toys. There are countless manufacturers of high quality wooden toys in the market. Everything from baby rattles to kitchen play-sets are now made out of wood. Some commonly available brands include Plan Toys, Baba, Turner Toys, Selecta, and Holztiger.

Choose safer food packaging and serving containers.

Avois polycarbonate plastic food containers. Check the bottom/underside of the porduct. If you see "PC" (usally in or near the recycling triangle) signifying polycarbonate plastic, do not purchase it. Often a number "7" on the bottom in the recycling triangle, by itself, also means the material is polycarbonate, but not always. To be safe, avoid #7 plastic. Choose plastics labeled #1, #2, or #5 in the recycling triangle, but do not heat beverages or food in plastic containers of any kind.

Avoid PVC plastic in food containers. Check the bottom/underside of the product. If you find the number "3" in the recycling triangle, it is made from PVC plastic and should be avoided. Avoid canned food: Unfortunately, bisphenol A can leach from metal can lining into the food and liquids contained within. Buy baby food in glass containers, avoid feeding your child food from cans as much as possible. You can foten find popular children's food, such as tomato sauce, applesauce, and black beans, in glass jars.

Choose safer containers for sippy cups and water bottles. Look for plastics labeled #1, #2, or #5 in the recycling triangle. As an alternative to hard plastic water bottles ( Such as the polycarbonate Nalgene bottles), try a lightweight stainless steel bottle instead.

Choose glass or safer -plastic baby bottles, Almost plastic baby bottles are made from polycarbonate plastic containing bisphenol A, but they are rarely labeled as such. With as few as 500-100 washings- even before you see wear- significant amounts os bisphenol A can leach into your baby's milk. For the best protection, switch to using glass bottles for all or most of baby's use.        Contrary to claims by the plastic industry, glass bottles are extremely durable and safe (and wash well in the dishwasher). And after all, they were good enough for you when you were a baby! Evenflo is one of the only glass bottle makers around (some Babies "R" Us stores carry them and they are available on-line). A couple of manufacturers make their baby bottles from a safer polycarbonate- based plastic (a softer, opaque plastic)< which has not been associated with the developmental problems linked to bisphenol A.

Choose metal feeding utensil and enamel or ceramic plates. While many manufacturers have removed phthalates from products intended to be put into young children's mouths, without prohibiting their use, there is no guarantee that these products, such as soft, plastic-coated feeding spoons, are made without phthalates. Look for PVC-Free labels or buy stainless steel, enamel, ceramic, or glass. (Note that enamel cannot be put in the microwave, and you should not use old pottery that could have lead-based glazes).

Avoid foods wrapped in plastic. Almost all commercial grade plastic cling wrap contains PVC plasticized with phthalates, and other plastic food packaging may be made of PVC, as well. Avoid buying foods wrapped in plastic especially cheeses and meats. Buy deli-sliced cheeses and meats and have them wrapped in paper. If you can't avoid buying plastic-wrapped foods, cut off a thin layer of the cheese or meat when you get home and store the remainder in glass or a less-toxic plastic.

At Home

Use glass to heat food or liquid in the microwave. You should not heat food in plastic containers or on plastic dishware, or heat liquids in plastic baby bottles. Heating food and liquids in plastic containers can cause chemicals and additives in the plastics to leach out more readilt-right into baby's food and milk. While some plastic containers are marketed as "microwave safe," it is safest to avoid them for heating.

If you do use plastic bottles, containers, or dishware, avoid harsh detergents or hot water when washing them to reduce exposure. Don not put plastic bottles, containers, or dishware in the dishwasher. Also, throwout any plastic bottles, containers, and dishware that start to look scratched or hazy. Do not let milk sit for long periods of time in plastic.

Avoid letting your child put plastic toys in his/her mouth. Toys designed for older children are more likely to contain phthalates or bisphenol A. It i s assumed that young children will not mouth these toys- such as action figures and Barbie dolls. To be safe, keep all plastic toys out of children's mouths. Call the maufacturer if you want to know if a product contains phthalates or bisphenol A.

Struggle with Anxiety and the "Treatment"

I have been with my fiance for 2 1/2 years. He has suffered from Anxiety and Panic Disorder since 2001 before I knew him. He has been tossed around from doctor to specialist to therapist to psychiatrist. NOTHING they have done has worked for his disorder. Every medication he has been on came with more side effects and doubts it will ever go away. I have tried my best before to understand and to have patience with it. We were almost at the end. What is a Panic Attack?

panic attack can only be described as a comprehensive emotional nightmare. Some people with panic feel like they are in an escalating cycle of catastrophe and doom and that something bad is going to happen to them "right now this very moment."

Products marketed for infants and children are not always completely safe for their use. Many contain toxic chemicals that may have detrimental health impacts for children exposed during critical stages of development. In this report, we analyze the extent to which five popular brands of baby bottles leach bisphenol A, a developmental, neural, and reproductive toxicant, into liquids coming into contact with them.  We found that all five brands leach bisphenol A at dangerous levels found to cause harm in numerous laboratory animal studies.

California and the U.S. should reform chemical policy to ensure that all products on the market are safe for children.

Bisphenol A is a Developmental, Neural, and Reproductive Toxicant

Scientists have linked very low doses of bisphenol A exposure to cancers, impaired immune function, early onset of puberty, obesity, diabetes, and hyperactivity, among other problems.For example, in one recent study, a single, low dose of bisphenol A administered to a newborn rat resulted in hyperactive behavior.Exposure to Bisphenol A is Widespread

Bisphenol A is most commonly used to make clear polycarbonate plastic for consumer products, such as baby bottles. Through use, this plastic breaks down and leaches bisphenol A into liquids and food to which it comes into contact.The U.S. Centers for Disease Control and Prevention found bisphenol A in the urine of over 95% of people they tested.Alarmingly, the median level of bisphenol A in humans is higher than the level that causes adverse effects in animal studies.

Based on a consumer survey of the most popular baby bottle brands on the market, we selected five bottle types to determine the amount of leaching from each bottle. Recommendations for Parents

Parents have the right to know about chemicals in the products they purchase for their children. In the absence of good government regulations, but armed with the knowledge that some chemicals are a cause for concern, parents can take a few simple actions to limit their child’s exposure to these and other toxic chemicals.

At the store, parents should select baby bottles that are made from glass or a safer non-polycarbonate plastic. At home, parents should avoid washing plastic dishware with harsh dishwashing soap and hot water, which may allow chemicals to leach out of the plastic. Recommendations for Policymakers

Parents cannot deal with these issues alone. The government must ensure the safety of all products on the market for children.
California and the U.S. should:

Phase Out Hazardous Chemicals

Based on the weight of the scientific evidence showing the harm caused by exposure to bisphenol A, the government should act now. Given that data from the U.S. Centers for Disease Control and Prevention show that bisphenol A is present in humans at levels found to be harmful in laboratory studies,
California and the U.S. should phase out the use of bisphenol A, especially in products used by children.

Inform Consumers about the Presence of Dangerous Chemicals

Parents currently have little information to inform their decisions when purchasing products for their family. Manufacturers should be required to label children’s products with the name of any potentially dangerous chemical and the specific health risks associated with the chemical.

Reform Chemicals Policy

Currently, manufacturers can put chemicals on the market without proving they are safe. Chemical manufacturers should be required to provide all hazard and health-effects information to the government so agencies can begin to assess the thousands of chemicals currently on the market for which little or inadequate data are available. Next, pre-market hazard and health-effects testing should be required for all new chemicals before they are introduced into commerce. Finally, the
California Environmental Protection Agency must have the authority to protect public health by banning or restricting the use of a chemical if evidence shows that it can harm human health.

There is growing debate about the potential health implications of the chemicals that leach from some plastics, and especially their possible effects on babies. You may have seen headlines raising concerns about the soft vinyl teethers and toys that infants sometimes suck or chew. The Consumer Product Safety Commission has, as a precaution, recommended that parents dispose of any such items they own. Based on our own findings, we think the agency's advice is sound.

But soft vinyl isn't the only plastic used in baby products that may contain chemicals that have caused harm to lab animals. We also found that one such chemical leached into simulated infant formula when we heated it in one type of clear plastic baby bottle.

Parents whose infants use plastic bottles and teethers shouldn't panic.
It isn't yet known what risk, if any, the chemicals that can leach from some of these items may represent to humans. However, until more is known about their effects, it makes sense to limit exposure to them. That's especially wise when it comes to infants, who could be at highest risk.

Here's a rundown of our findings, and advice on what they may mean for
parents:


Teethers and soft toys

Mainly at issue here are items made from polyvinyl chloride (PVC), commonly known as vinyl. PVC's soft texture is imparted by "plasticizing" chemicals. The plasticizer most widely used in teethers is DINP, a member of the phthalate family of chemicals. Administered to lab animals at high doses, DINP has caused cancer and damaged the liver, kidneys, and other organs.

PVC is used for a range of soft toys and other items for infants, but the greatest concern is with teethers, since those are specifically designed to be "mouthed"--and the chewing action can break down the plastic, accelerating the release of chemicals. (Pacifiers and feeding-bottle nipples are made of latex or silicone, and so are not of concern.)

Six of seven widely available teethers we bought and tested last December were made of PVC and contained DINP. The Consumer Product Safety Commission has assured parents that "few, if any, children are at risk" because the amount of DINP they may ingest from mouthing PVC toys is well below the level believed harmful. However, the commission also said more studies are needed to fully assess DINP's possible effects on infants.

The Vinyl Institute and the Toy Manufacturers of America, the respective industry trade organizations, say that PVC toys and teethers, which have been used for 50 years, are safe. Still, most toy makers have indicated that they'll voluntarily comply with the commission's request to reformulate those products and switch to less controversial materials--including polyethylene or EVA plastic, which contain no plasticizers.

However, PVC teethers and baby toys remain on the shelves in some stores. Gerber, a major manufacturer, says it has withdrawn all phthalate-containing teethers from stores. However, most other manufacturers have pledged only to stop making any more such items. Since we bought our teethers, some major retailers, including
Kmart, Sears, Target, Toys "R" Us, and Wal-Mart, have removed such items, and will now stock only products that manufacturers vouch for as phthalate-free. But in early March, our shoppers confirmed that many of the PVC teethers we bought last year remained on the shelves at some retailers, such as major drugstore chains.

Baby bottles
The question here focuses on baby bottles made from polycarbonate, a clear and rigid plastic. We bought six different bottles and heated plastic from each in simulated infant formula. The plastic from each of the bottles leached into our test formula a chemical called bisphenol-A, which in lab animals has produced physiological effects similar to those produced by estrogen. During such "endocrine disruption," chemicals interfere with or mimic the action of hormones, possibly upsetting normal development. (Subscribers, see our June 1998 report, which included tests for chemicals that can leach from certain plastic wraps into fatty foods.)

Based on testing with an intact bottle, we calculate that a typical baby who drank formula sterilized by heating in the bottle would be exposed to a bisphenol-A dose of about 4 percent of an amount that has adversely affected test animals in studies by Frederick vom Saal, professor of biological science at the University of Missouri, Columbia.

Such exposure may sound very low. However, safety limits for infant exposure can be set as low as 0.1 percent of the level that has adversely affected animals. Babies who used the bottles we tested could be exposed to a bisphenol-A dose 40 times higher than that conservative definition of safety.

George Pauli, the director of the division of product policy at the
Food and Drug Administration, said the agency has looked into bisphenol-A leaching, and it stands by its decades-old approval of polycarbonate baby bottles as safe. We think the FDA needs to re-examine the issue in light of our data and recent concerns about the sensitivity of babies to the estrogenlike effects of chemicals such as bisphenol-A.

Recommendations

Teethers. The Consumer Product Safety Commission has created a Chronic Hazard Advisory Panel, which the agency hopes will report in about a year on the safety of PVC, phthalates, and alternative substances. Meanwhile, we recommend that parents of young children follow the commission's advice to dispose of all PVC teethers and soft toys used by infants "as a precaution," and to replace them with items that are phthalate-free.

To increase your chance of finding phthalate-free items, shop at one of the stores that have pledged to stop selling heavily mouthed baby products with phthalates, including
Kmart, Sears, Target, Toys "R" Us, and Wal-Mart.

If you shop elsewhere, avoid using the item until you call the manufacturer's toll-free number you'll find on the product packaging. Return the item to the store if it isn't phthalate-free.

Baby bottles. Just to be safe, to avoid any possible exposure to bisphenol-A we advise parents of young children to dispose of polycarbonate baby bottles and replace them with bottles made of glass or polyethylene, an opaque, less-shiny plastic that does not leach bisphenol-A.

Unfortunately, plastic baby bottles do not explicitly indicate that they're made of polycarbonate. But there are some ways you can tell; see "What to Do," below. Or you could call the manufacturer's toll-free number, listed on the package.

It shouldn't be this difficult for parents to make an informed choice about plastic items for their babies. The Children's Environmental Protection and Right to Know Act, a welcome bill introduced in May 1998 by
Rep. Henry Waxman , D-Calif., would require disclosure to the government of potentially toxic chemicals used in children's products. If implemented, that requirement would eventually allow parents to consult, say, a government web site for information on the composition of plastic toys. Right now, it would help if manufacturer's web sites included this information.

What to do / Precautionary steps

Teethers

Dispose of: All teethers and heavily mouthed toys made of soft plastic.

Replace with: New items that manufacturers confirm are made from plastics that don't contain phthalates. Call the toll-free numbers usually found on packaging.

Bottles

Dispose of: All clear, shiny plastic baby bottles, unless the manufacturer tells you they're not made of polycarbonate.

Replace with: Bottles made of glass or an opaque, less-shiny plastic (the plastic bottles are often colored).

Glass baby bottles, replaced decades ago by unbreakable plastic, are making such a comeback that parents can't get their hands on them.

Online and traditional retailers report a run on glass baby bottles in recent weeks that they say was spurred by reports that the most common type of plastic in baby bottles may leach a toxic chemical.

San Francisco's Sean Mullins said he decided to switch his 6-month-old son, Mickey, from plastic to glass bottles last month despite manufacturers' insistence that plastic bottles are safe.

"You want to avoid anything that could be a health risk to a baby," Mullins said. "You try to give them the best start." But after searching stores, checking Web sites and being outbid on
eBay, "we could not find them anywhere."

A friend finally found some glass bottles after searching online for more than two hours, but they were back ordered and aren't expected to be shipped until late this week.

Independent tests done for the
San Francisco Chronicle and reported in November found bisphenol A, a chemical that mimics estrogen, in a baby bottle and several toys.

Bisphenol A is also found in the lining of food cans, some anti-cavity sealants for teeth and electronics.

Then, in late February, Environment
California, an advocacy group, released a report titled "Toxic Baby Bottles" that drew intense national media coverage.

When heated, five of the most popular brands of polycarbonate -- the clear, shatterproof plastic used in baby bottles -- leached bisphenol A at levels that have been found to cause harm in laboratory animals, Environment
California found.

Even at low levels, bisphenol A has been linked to abnormalities in the mammary and prostate glands and the eggs of laboratory animals, scientists say. Animal tests also show that bisphenol A can speed up puberty and add to weight gain, and may cause changes that can lead to breast and prostate cancer.

"Parents are so concerned," said Dan Jacobson, Environment California's legislative director.
